There are two types of implant crowns: abutment supported and screw retained. When submitting a claim for an abutment supported crown please add the abutment code. Abutments are not needed for screw retained crowns. Therefore, if an abutment is received then a screw retained crown submitted, it will be automatically denied. Abutments are not needed for crowns coded as D6065, 6066, 6067, 6075, 6076, 6077, 6082, 6083, 6084, 6086, 6087, 6088, 6098, 6099, 6102, 6131, 6122, 6123.

D6065 implant supported porcelain/ceramic crown. A single crown restoration that is retained, supported and stabilized by an implant. **for this code a separate abutment code is not allowed.

D6058 abutment supported porcelain/ceramic crown. A single crown restoration that is retained, supported and stabilized by an abutment on an implant. **for this code a separate abutment code is allowed.
